Viewing your Public Profile

1. Tap the Profile button in bottom navigation.

2. To view what your healthcare provider can see, tap ‘View Public Profile’

Editing your Personal Info

1. Tap ‘Personal Info’ in ‘Profile’

2. Tap the information you wish to change. (Note: you cannot change any information in grey.)

3. After typing in the new information, tap ‘Update’ to save.

Editing your Address Info

1. Tap ‘Address’ in ‘Profile’

2. Tap the information you wish to change. (Note: you cannot change any information in grey.)

3. After typing in the new information, tap ‘Update’ to save.

Adding, Editing, and Deleting Documents

1. Tap “My Documents” in “Profile"

2. To add an insurance card, tap here:

To add an ID card, tap here:

3. Type a name for the document.

4. For each of the front and back sides, tap “Add image” to open the camera and take a picture.

To delete a document, tap the document you want to delete, tap the three dots, and tap “delete document.”

Adding and Editing Dependents

A dependent is anyone who shares an insurance with you and/or cannot make a Teleport MD account by themselves. This may include children, spouses, and parents.

1. Tap “My Dependents” in “Profile”

2. Tap “+Add Dependent"

3. Enter your Dependent’s information.

4. Tap ‘Continue’ to save the information and move to the next page.

You can add documents for your dependents. (See Above)

Once your dependent is created, you can make appointments for them as if it were their account.
